benefit program that allows you to select the plans and coverage

All regular full-time employees are
eligible after 90 days of employment. We offer several options:
- Copay Options
a PPO plan that provides comprehensive medical coverage under
two options, the Copay and Copay 500 options. Both plans allow
participants to be wise consumers of medical care through the
use network providers, and include the use of deductibles, coinsurance,
and out-of-pocket maximums for inpatient and outpatient services,
and co-pays for prescriptions and office visits.
- HMO available at designated
locations in California, Florida, Georgia, Hawaii, Indiana, North
Carolina, Ohio, Pennsylvania, South Carolina, Tennessee, Texas,
and Virginia.

All regular full-time employees are
eligible after 90 days of employment, when they enroll in a Copay
or HMO option. Benefits vary by group health option.

All regular full-time employees are
eligible after 90 days of employment. The plan covers preventive,
basic, major, and orthodontia dental care.

All employees are eligible for discounts
on eye exams, lenses, frames, contacts and LASIK surgery through
participating providers.

All employees are eligible after 90
days of continuous employment. Save from 1% to 50% of your eligible
compensation not to exceed $11,000 annually (as indexed) on a tax-deferred
basis. Lowes will match your contributions after one year of employment.
After three years of employment, an additional Performance Match
may be earned based on company performance.
